Congratulations on the decision to make Genesis your health care provider! Please fill out the following intake form and watch for an add friend request from LadyBree. You will have an opportunity to chat with me and ask questions. If we agree that Genesis services are right for both of us, you will need to have a PayPal account to put down a minimum 50% of your balance due.  I will message you to make an appointment for your intake exam. Your balance must be received before your first appointment time. This exam will take approximately 30-40 min. Most other visits will take about 15 - 20 min. You will need to bring a hospital gown and slippers with you to your visits.
